This is the
Registration form (pdf).
You can fill this in if you want to be able to vote.
To fill in the form you must use a black pen and block capital
letters.
It is fine for someone else to fill in your details, but you
must sign it yourself.
This does not have to be a full signature. Whatever your usual
mark is will be fine.
Section 1 - About You
You must write your surname, your first names - all of them -
then your nationality. For lots of people using this website this
will be British.
The next bit you only fill in if you are 16 or 17. In this case
you must give your date of birth.
Finally in section 1, put a tick in the little box if you are 70
years old or more.
You don't have to do this, if you live in Scotland.
Section 2 - Full Postal Address
You must write down your address. On the bottom line put your
postcode.
Under the postcode there is a blue box. We recommend you tick
the little box that's here, as this means you will not go on what
is called the Edited Register which can mean junk
mail coming through to you which you don't want.
You do not have to give your phone number or email address if
you don't want to.
Section 3 - Previous Address
You do not have to fill this in if you have never registered to
vote before.
At the end of section 3, if you would like to find out about
voting by post, tick the first little box.
If you would like to find out how you can arrange for someone to
vote on your behalf, tick the second little box in section 3.
Section 4 - Declaration
You must sign the form in this section with your usual mark, and
put the date under it.
If you have followed my instructions, you have completed the
form.
To find out where to send your form to, go to the link on the
text www.aboutmyvote.co.uk. This will take you to
the 'About my vote' website. Enter your postcode in the box and
click on the 'Go' button next to it. The next page will tell you
the name of your local council. This is where you need to send the
form. Go down the page to find its address. This is in the bottom
right-hand corner.
